Follow Up Visits
- Identify and address safety concerns (fall prevention)
- Establish care and ensure that it meets individual patient needs
- Teach measures to manage health condition(s) and medical symptoms
- Review medications and discuss all changes in medication
- Provide patient and/or caregivers with strategies to improve function and independence
