The future of healthcare is at your door! A person is in their most vulnerable state when faced with a new or ongoing health condition and is navigating the emotions and stress of a fragmented healthcare system. At Transitions, we are here to help! I am Dr. Mark Sarinopoulos, the proud owner of Transitions Healthcare. It will always be in my nature to advocate for all of my patients on all fronts.

Throughout my eighteen years of experience in private practice and as Chairman/CMO and Medical Director for two independent providers, I have witnessed firsthand the gaps in our healthcare system and the tangible consequences that these gaps have on our community, patients, families, and friends.

In 2013, I was inspired to build both a home healthcare company and palliative care company to help address the shortfalls in healthcare. Later, I added an outpatient therapy program to complement the two existing companies. This trifecta offers a seamless continuum of care for patients as they manage their conditions. Through this model, I proactively address and redress the care errors caused by our fragmented healthcare system and prevent the decline of a patient’s condition before a 911 event occurs.

During the care process, I approach each patient with a fresh pair of eyes. I talk with patients and their care teams to treat the whole person and restore physical, emotional, and/or mental health. Each patient has a unique history and requires a tailored plan, compassionate care, and a team dedicated to success. My team is hand selected and vetted to ensure they have the passion and compassion to treat patients according to my model and ideals.

My services are not confined to a single place and give the flexibility of treating patients compassionately in the hospital, skilled nursing facility (SNF), or at home. I also stay up to date with the latest innovative trends, treatments, and technology in order to provide consistent quality care.
